I have experience in working with adult and pediatric patients for over 10 years. Currently, as a practicing physician, he works mainly in Primary Health Care Clinics, caring for thousands of patients for many years. It provides patronage, check-ups on healthy children, mandatory and recommended vaccinations for children and adults, and admits sick children and adult patients with infections and many other diseases. I also take care of patients from Social Welfare Homes (DPS), Medical Care Facilities (ZOL) and support hospice patients. Telephone consultations for minors take place only in the presence of an adult guardian.

I am exhibiting:
1) E-prescriptions (it is possible to receive a reimbursed prescription - after meeting the reimbursement criteria, currently it is not possible to receive an "S" prescription, the so-called 65+, and a DZ prescription, the so-called 18-, due to the limitation in the regulations for commercial = private visits)
2) E-certificates (including exemptions for foreign employers - on the ZUS-ZLA form, additionally, as part of the visit, we print the e-certificate, sign and stamp the electronic document, then send the scanned copy by e-mail to the patient - scans of the exemptions are sent at the end of the day working day - we recommend checking e-mail, including spam - the day after the visit)
3) care for a sick child (on the ZLA form)
4) Telephone/personal consultations in the case of minors take place only in the presence of an adult guardian (patients aged 16 and 17 may come for examination without a guardian, only after providing consent from an authorized person - we suggest sending the consent to the e-mail address or taking personally).
5) E-referral for X-ray, MRI, CT, colonoscopies, gastroscopy and others - as part of commercial tests (PAID)
6) medical certificates - for a driving license category AM, A1, A2, A, B, B1, B, B+E, T - choose the type of visit "examination of drivers category A, B" or examination of drivers - office at ul. Dąbrówki 92/7 - tests for new drivers and people renewing their driving licenses.

I do not issue:
1) I do not continue to treat patients taking sleeping pills, sedatives, psychotropic drugs or drugs prescribed by a psychiatrist. I do not issue prescriptions for RPW (narcotic drugs). Patients under the care of the Mental Health Clinic are asked to contact their attending physicians.
2) I do not prescribe very strong pain medications, especially to patients who are at risk or suspected of addiction.
3) I do not issue invoices - the document confirming payment is a transfer document - in accordance with the provisions of Polish law regarding the provision of distance advice.
4) I do not issue certificates on the lack of contraindications to practicing sports, work, etc., or other certificates sent by the patient.
5) We do not make calls to foreign numbers - in such a case, please make an e-visit via chat to contact the doctor
